Home Infusion Services
The home infusion team consists of the:
- Physician – orders the medication and evaluates its effectiveness through lab tests and input from the pharmacist and nurse
- Pharmacist – prepares the prescribed medication and ensures that the medication, infusion pump and supplies are delivered to the right patient, at the right time, in the right dosage
- Nurse – teaches the patient/caregiver how to infuse the medication based on the method selected so that the patient can become independent
We provide:
- Patient/caregiver training by highly specialized infusion nurses encourages active participation in the therapy and understanding of the therapeutic goals while still in the hospital
- Coordination of an overall home care plan of infusion therapy
- Emotional support and counseling to ease the transition to home
- Infusion solutions compounded to precise specifications using state-of-the-art equipment
- Provision of technologically advanced equipment, supply delivery, and careful inventory control to ensure that patients have appropriate supplies on hand at all times
- Clinical monitoring of infusion patients
- Nutritional assessments performed by registered dietitians on TPN and enteral patients
- Monitoring of the patient’s clinical status and treatment efficacy as well as patient compliance and tolerance of the therapy
- Identifying and communicating in a timely manner to the physician any related problems or reactions to permit appropriate medication adjustments
- Processing of claims to Medicare, Medicare HMOs, Medicaid, private insurance companies, and others
